This letter contains helpful information about our practice, such as our philosophy and range of services, how to get here, parking options and public transport connections. In addition, you will receive a medical history form, which will enable us to better understand your medical history and individual needs.
We would like to give you the opportunity to fill out the medical history form in a relaxed atmosphere at home and then bring it with you to our practice at the agreed appointment. We are already looking forward to seeing you!
